3. The notice set out in section 443 of the Act to inform the client of his right to choose an insurer or a representative of his choice when a distributor requires that the debtor purchase insurance to guarantee the reimbursement of his loan shall comply with the model set out in Schedule 2 - NOTICE OF FREE CHOICE OF INSURER.
